Both the Default Exchange Fee and Self-Service Exchange Fee are charged per exchange transaction.

Manually, yes! If you want the Exchange Fee to be higher for certain exchanges, such as exchanging tickets the day-of an event, users can manually update the Exchange Fee field when processing the exchange.
If you'd like the ability to set conditional Default Exchange Fees, make sure to submit a Product Idea!
Not at this time. If you'd like the ability to set Self-Service Exchange Fees on a Ticketable Event or Event Instance level, make sure to submit a Product Idea!
